Clinical Study Informed Consent
This template should be used for any clinical study in which patients will be evaluated at the VTH. Information in red italic should be edited to include information specific to the study and/or patient. A completed consent form should be submitted to the CSC coordinator (ccmtr_csc@ncsu.edu) for approval by the CSC Steering Committee prior to commencement of the study.
Clinical Study Communication Form
This form should be completed and submitted to the Associate Dean of Veterinary Medicine Services prior to initiation of any clinical study that utilizes hospital resources.
