Adobe PDF Instructions:
THE CVM RESEARCH OFFICE HAS BOUGHT
A 10 SITE LICENSE TO ADOBE ACROBAT. THIS MEANS THAT 10 PEOPLE
AT THE CVM CAN USE THIS PROGRAM CONCURRENTLY. ONCE 10 PEOPLE
ARE LOGGED ON THEN THE 11TH etc. WILL BE NOTIFIED OF THE SLOTS
BEING FULL TO TRY AGAIN LATER. WE ASK THAT IF YOU RUN INTO
THIS OFTEN PLEASE LET OUR OFFICE KNOW SO THAT WE CAN MAKE
PLANS TO INCREASE THE NUMBER OF LICENSES.
These
forms are in PDF format. This format is interactive (i.e.,
you can type on the form and then print. There are two ways
to save the file once you are done.
1.Type on the form while
on the web and then save the file to your hard drive with
a new name.
2. Save a clean copy onto your
hard drive and type on it later in Acrobat. If you do this I
recommend once you type your data in then do an export of form
data and then close the file. When you need to update or change
then reimport the data from the previous file you just created.
When you have the forms open, click on the page and your cursor
will blink in the first active field. The tab key takes you
to the next active field on the page (s). Boxes are check boxes
and can be activated/ deactivated with your space bar or mouse
(if you do not want to check a box use the tab key (or mouse)
to skip and go on to the next field). Date fields should be
typed in the following format 00/00/00 . Currency fields
will automatically put the neccessary punctuation in for you.
