How to Schedule an Appointment
We would like to work with you to help solve your pet's behavior problem. Please follow the instructions below if you wish to set up an appointment for a dog or cat-related problem.
- Fill out a behavioral history form for your pet. Click on Behavioral History Forms to download and fill out either the Canine Behavioral History Form or the Feline Behavioral History Form.
- Send the form to us via U.S. mail (preferred) or fax (919-468-6338). Because of security issues, please DO NOT e-mail this form to us. To reserve a two-hour appointment, be sure to include with your history form a credit card number with $50 payment authorization or a check (payable to the NCSU Veterinary Teaching Hospital) for $50. Note cancellation policy, below.
- After sending your history form, please call the Animal Behavior Service (919-460-8512) to schedule your appointment. If you leave a message, we will return your call when we are next in the office, generally on Mondays and Thursdays.
- Our fees for the initial consultation are determined by the amount of time that we spend working with you and your pet(s), charged at $180.00 per hour. Generally, feline cases require 1.5-2.5 hours ($270-$450); most canine cases require 2.0-2.5 hours ($360-$450). We accept personal checks and most credit cards.
- Cancellation Policy - Since we reserve a two-three hour appointment for you, we appreciate the courtesy of early notification if you wish to cancel your scheduled appointment. Your appointment reservation fee of $50 is fully refunded without penalty if you provide 48 hours notice of cancellation prior to your scheduled appointment.
